What is a Nexus Letter?
A Nexus Letter is a medical document written by a qualified healthcare professional, such as a PA-C or Nurse Practitioner, that provides a detailed, evidence-based opinion connecting a veteran's current medical condition to their military service or to an existing service-connected condition.
It typically includes a review of the veteran's medical and service records, references to relevant medical research, and a clinical rationale that supports the likelihood of a service connection. This letter is used to strengthen a VA claim by clearly establishing the "nexus" between the condition and military service.
Are Nexus Letters always provided?
No, we cannot guarantee a Nexus letter from the Medical Nexus team. The Medical Nexus team will review the request and uploaded documents, then communicate with the veteran directly through the patient portal. Documentation is essential, and the Medical Nexus team cannot complete Nexus documents without the proper documentation.
What is a DBQ?
A Disability Benefits Questionnaire (DBQ) is a form created by the VA to help document the severity of a specific medical condition. It's used to support your disability claim by aligning your symptoms with the VA's rating criteria.
There are two main benefits to a DBQ:
1. It highlights how severe your condition is based on how the VA assigns disability ratings. For example, conditions like migraines have specific questions on the DBQ that can support a rating of 30% or higher.
2. It may help you avoid a C&P exam. If your DBQ and supporting evidence meet the VA's requirements for a fully developed claim, the VA might make a decision without needing to schedule an additional exam.
In short, a DBQ helps clearly communicate how your condition impacts you in a way that aligns with how the VA makes rating decisions.
Do you complete DBQs for all conditions, or only certain ones?
We can only complete DBQs for conditions that do not require an in-person physical exam for accurate assessment. Some conditions—like orthopedic issues or most neurological conditions (with the exception of headaches)—typically require a physical evaluation to meet VA rating standards. For conditions that can be assessed through medical history and symptom reporting, we're able to complete the DBQ remotely.
What is a P&T letter?
A P & T (Permanent and Total) letter is a document available to veterans who are already 100% service connected. It confirms that their service-connected conditions are considered static, meaning they are unlikely to improve and may even worsen over time. One of the primary advantages of this status is that it exempts the veteran from routine re-evaluations by the VA and may provide additional benefits for both the veteran and their family. Veterans interested in obtaining this document should sign up for the medical Nexus service for review. Additionally, maintaining up-to-date treatment records (from the past 1-2 years) for each service-connected condition is highly recommended.
